New Hire Checklist

Use this as a checklist (if needed) to ensure all new hire forms are turned in.

Background Checks and Health Forms

Step 1: Make an appointment within 24 hours and obtain Fingerprints

To schedule an appointment, visit: https://fieldprintvirginia.com/ and create a new user account.

Under Fieldprint Code enter: FPVADSSChDayCntrApp
Enter your Personal Information. Note: your name and date of birth must match your ID.
Under Facility ID enter: 116712
Once your fingerprint appointment is scheduled, click here to notify our team. (Replace this link)
Remember: You must take 2 IDs to your appointment.
We will contact you to start work once FieldPrint emails your results (3 to 5 days).

Step 2: Complete VA DSS Sworn Statement

Step 3: Complete Virginia Registry Form (must be notarized)

A $10 money order made payable to Virginia Department of Social Services should accompany this form.
Most banks will notarize your form for free.

Step 4: If you have lived in another State within the last 5 years

If you have lived in DC within the last 5 years, please also complete that form.
If you have lived in MD within the last 5 years, please also complete that form.
If you have lived in another State (besides VA, MD, or DC), review the provided link to obtain the form.

Step 5: Obtain TB Test Screening (Due by Day 14 of Employment)

Note: The CVS Minute Clinic behind our Leslie Avenue location offers TB testing.

Step 6: Complete Tax and Financial Forms Packet (I-9 Form, W-4 Tax Form, Direct Deposit Form)

Step 7: Fully complete Employment Application

Step 8: Provide certificate of completion for free online child abuse course

Step 9: Fully review Staff Handbook and complete Employee Agreement Forms
